I confirm I am authorised to bind the pharmacy and intend my typed name and submission to be an electronic signature. I understand the pharmacy remains controller for patient queue data and must complete its lawful-basis, DPIA, privacy-notice, retention, staff-access, and related decisions. I understand registration does not authorise live patient use. The pharmacy privacy and go-live approvals must be completed before public check-in is enabled.
